How To Use this Website
 
First-time Visitors

Create a New Account

1) Create a new user account by confirming your age and clicking the start button on the home page.

2) You will be taken to the Create an Account page. Complete the form, and then click Register. Note that a Username can be any combination of letters and numbers, not spaces or special characters.

3) Upon successful registration of your new user account, you will be directed to the Manage My Living Will page. To begin your living will, click Start Your Living Will button.

Returning Visitors

Accessing Your Living Will

1) If you have already created a TexasLivingWill.org user account, you can sign in by entering your Username and Password into the Returning User Login form.

2) Upon successful sign in, or if you elected to have the site remember you during your previous visit, you can access your Living Will by clicking Your Living Will, either in the top right hand column or on the home page.

3) You will be directed to the Manage My Living Will page. Click the Edit Your Living Will button.

Creating / Editing Your Living Will

Filling Out Your Living Will

  1. Complete or edit your living will by reading and filling out the forms you encounter during each step of the process.
  2. Save each step of your living will by clicking the Next button on the bottom of the page.
  3. You may navigate to any step by using the numbered links at the top and bottom of the page.
  4. Find definitions to the terms used in your living will by clicking the more information icons.

Printing and Saving Your Living Will

  1. Upon completion of your living will, you will be returned to the Manage My Living Will page.
  2. You will now have the ability to print and/or save both of your Directive to Physician & Family and Medical Power of Attorney documents.
  3. Click Print under the document type you want to print and/or save. You will be prompted to either Open or Save the document to your computer.
