NOTE:
TexasLivingWill.org features a VIEW ONLY function for individuals
to view your unsigned advance directive, but not make changes
to it. By checking this box, you agree to enable this
feature as well as to use your driver’s license number (or
state identification number) as an identifying code. A person
entering your driver’s license number (or state identification
number) will be able to view or print an unsigned copy of your
advance directive. The view only version will not have a signature.
You must print out, sign, and have it witnesses or notarized to
create a legal document. You must sign your advance directive
to make it legally valid. This function also allows your doctor,
medical power of attorney, family, and even friends to see your
unsigned advance directive without being able to make any changes.
You can make changes to your living will at any time by entering
the site with your Username and Password. |
